pressures
hi i have pds and wondered at what pressure ranges do they start treatment for glaucoma. been going every 3 mths last time was 28 in right eye and 22 left which has increased by 2 each eye for last year every 3 mths. even though it had increased too 28 my consultant told me as the drops are so expensive and i would be on them for life she will re test in 6 mths, now been 5 mths and i have headaches everyday should i insist in going before the 6 mths or wait.. also found a scar too the back of my retina last time, consultant said was nothing too worry about as inactive then sent letter too my doctor expressing concern..? |

Hi Jackie, The decision to start treatment for ocular hypertension (Intra Ocular Pressure being raised without evidence of damage) can be a tricky one. Firstly, there is the matter of whether the IOP is what we think it is. The thickness & properties of the cornea (the clear window at the front of the eye) can lead the measure to be artificially high or low. But I would have thought that this would have been measured & mentioned to you if it was abnormal. Secondly, it depends on the risk of both you getting any Glaucomatous damage (damage to the optic nerve which may be prevented by reducing IOP) & also of suffering visual impairment as a result. This depends on your age, the appearance of the optic nerve head of your eye, how you perform on the visual field test, other systemic or ocular conditions you may have and, as you know, IOP. Thirdly, although I'm sure a NHS consultant has no real concern about the price of drops, they do have side effects, both social & medical. Once you are on them, you are indeed on them for life. This is inconvenient as well as increasing the risk of you developing cataract as well as other problems. So it perfectly sensible to practice caution before starting treatment for ocular hypertension. Some protocols have been developed which estimate the percentage risk of progressing to damage with & without treatment, which are often helpful. But many would have little problem with confirming the start of damage before instigating pressure lowering therapy. It is also handy to remember that even when damage has occurs, it often progresses very, very slowly. With regard to your headaches, I would emphasise that there are many causes of headaches. And one caused by raised IOP is both rare & unlike any other headache. You would also swiftly notice the ocular effects which would be associated with it (Halos, possibly fixed pupil, red eye etc). I don't wish to minimise what you're feeling but it is unlikely it is PDS related. As for the scar at the back or your eye I don't think I can be that helpful. Old choro-retinal scars are not uncommon & are completely benign. They are unrelated to PDS. There are lesions at the back of the eye which are worrying, a few of which are more common in PDS but without more details it's impossible to comment. I would say that it wouldn't be the first time that a general practitioner had misunderstood an ophthalmologists report... Hope this helps. Feel free to post again if I have been unclear. |
